Medical and Dental Plans

Medical 1 plan with three levels of coverage depending on the healthcare provider you choose.

Dental   Choice of 2 plans.

Prescription Drug Plan

A co-pay prescription drug plan is provided through the hospital pharmacy with participation in the medical plan. $5.00 co-pay for generic, $25.00 for brand with no generic substitute and $35.00 for brand. Mandatory generic/formulary dispensing; 90 day refill option available with certain drug therapy classifications. $10.00 copay for generic, $50.00 for brand.

Must participate in medical plan to be eligible for pharmacy coverage.

Core coverage = (1x Base Salary) provided at no cost. Higher coverage is available for a nominal charge.

Core coverage (50% of monthly salary) for short and long-term disability at no cost. Higher coverage is available for a nominal charge.

Professional Liability/Malpractice Insurance

Provided through Catholic Health Partners Self Insurance Fund. Annual aggregate of $3,000,000. $1,000,000 per each occurrence.
